New Patient Registration
If you live within our practice area (see the Practice Boundary tab along this page) you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill.
You will need to complete a registration form (GMS1) and a health questionnaire (download here) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You will also need to provide 2 forms of ID - one photo (passport, drivers licence) and one address (recent bank statement, council bill, utility bill, etc). If you are registering any children under the age of 5, please bring their red book when dropping in the registration forms (along with ID).
All new patients over the age of five and who have a chronic condition are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Change of Address/Telephone Number
Please inform us immediately if you change your address or telephone number (including ex-directory numbers). Failure to do so may result in you being taken off our doctors’ lists by Oxfordshire Health Authority. Please remember that a change of address may also mean you are outside our practice area.
