GENERAL PRACTICE DATA FOR PLANNING AND RESEARCH - YOUR CHOICE

This practice is supporting vital health and care planning and research by sharing your data with NHS Digital and important changes are due from 1st September 2021.  To view ALL the relevant information - provided by NHS Digital - we ask that patients should please click here.

If you have further concerns then you may wish to contact your local MP  as this is a national NHS and government made decision and not one made at GP practice level.

To opt-out of your NHS information being shared you can click to use the national opt-out online system. This is the easiest and simplest way to do so. We expect the deadline to opt-out will be 25th August 2021.

Alternatively, you can complete and submit the form we have provided via our link lower down on this page. 

You can download, print, sign, scan and then send your form by email to alchester.medicalgroup@nhs.net or send your completed and signed form by post to Alchester Medical Group, Victoria House Surgery, 119 Buckingham Road, Bicester OX26 3EU. When we have received your form we will then be able to action your request.
