Introducing MedLink

For our patients who have a long-term health condition, you may have noticed that we have been using a different system to manage and review these conditions.

From April, all patients who are on a long-term health condition will receive a link via email, text message or NHS App when it is time for your review. If you do not have any of those you may get an automated phone call asking you to contact us to make an appointment. Please select the link and complete your review using a smartphone, tablet, or computer. You can also copy the link into a browser on a larger screen. These personalised reviews combine all common conditions into one annual check. Any other conditions will be reviewed as before.

Your answers are saved automatically. If you don’t finish in one go, just return to the link later. Feel free to ask a friend or family member to help.

List of Long-Term Conditions included in these online review invitations:

  • Asthma
  • Chronic Kidney Disease (CKD)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Coronary Heart Disease (CHD)
  • Diabetes (annual review)
  • Heart Failure
  • Hypertension
  • Mental Health
  • Non-Diabetic Hyperglycaemia (NDH)
  • Peripheral Arterial Disease (PAD)
  • Stroke/TIA

Patients who won’t be getting these online review invitations:

  • Housebound patients
  • Those with dementia
  • Those who are on end-of-life care
  • Those that have a learning disability
  • Those who are particularly older, so anyone over the age of 90 (and some that may be over 85)
  • Any other patients who have already expressed in the past that they are unable to receive and/or fill in online questionnaires

As mentioned above, we will either send these patients text, email, or letter invites asking them to make an appointment for a review.

 

After submitting your review, you may be asked to book an appointment (eg blood test). Once everything is complete, the practice team will review your answers and follow up as needed. You will be able to book your appointment either through the completed questionnaire, or we will send you a follow up link to book your appointment.

These review invites will be sent to you during your own month of birth (so if you have hypertension or any other long-term condition, and were born in June, you’ll get an invite sometime in the month of June).

You may find that your invite is not quite a year from your last review. As we transition to this way of working, there will be some patients who have had a review quite recently; we hope that you understand that we’d like you to have your reviews lined up in your month of birth, however if you feel you would rather wait or would just prefer to have your review later in the year, you can do this. You will have to contact us yourself to book this review appointment as we will not otherwise contact you.

Finally we understand that there are patients who will be unable to complete these forms online. We will always invite these patients either through text or email invitations, or via a letter invitation. 

We already have a list of patients who we won’t be sending these online review invitations to, but if you get an invitation to complete your review online and you are unable to fill these forms in, please let the Practice know. We will be able to add a code to your medical record to ensure you no longer get these online review invitations.
